United States Department of Labor Issues Updated Family Medical Leave Act Form

Overview: On September 4, 2018, the U.S. Department of Labor's Wage and Hour Division released new Family Medical Leave Act (FMLA) notices and certification forms.

Effective Date: Immediate

Details: The new Family Medical Leave Act notices and certification forms are now available on the DOL website.

Except for an updated expiration date of August 31, 2021, which appears in the top right corner of the forms, the new versions are identical to the expired forms. There are no substantive changes.

Action Required: Employers should update their FMLA procedures to ensure that the new forms are used going forward.
